Background. Adolescence is a period of life that includes the transition from childhood to adulthood due to significant changes, both hormonal, biological, psychological and social. Indonesia is in fourth place where 20% of teenagers have had premarital sexual relations. Negative impacts that can be caused by promiscuous sexual behavior include exposure to sexually transmitted diseases (STDs) and HIV/AIDS. Aim. To determine the level of students knowledge about casual sexual behavior as a risk factor for HIV/AIDS at Madrasah Aliyah Negeri 1 Mandailing Natal. Method. This type of research is descriptive observational with a cross sectional design and uses simple random sampling techniques. Primary data was collected using a questionnaire method using a questionnaire conducted in July at Madrasah Aliyah Negeri 1 Mandailing Natal. And validity was carried out using SPSS before the questionnaire was announced to respondents. Results. The majority of respondents' level of knowledge had sufficient knowledge, 115 people (58.4%), 72 people (36.5%) had good knowledge, and 10 people had less knowledge (5.1%). Conclusion. The level of students' knowledge about casual sexual behavior as a risk factor for HIV/AIDS at Madrasah Aliyah Negeri 1 Mandailing Natal as a whole is sufficient. For this reason, it is necessary to provide education through outreach or seminars to increase students' knowledge regarding casual sex as a risk factor for HIV/AIDS.
