Background: Background: Acute Respiratory Infection (ARI) is an acute upper or lower respiratory tract infection caused by bacteria, viruses or fungi. ISPA disease often occurs in toddlers and lasts about 14 days. ISPA disease is the main cause of under-five mortality worldwide, including Indonesia. North Sumatra is the province with the highest ISPA cases, to be precise in the Simalungun Regency area (North Sumatra Health Profile, 2018). One of the factors for the occurrence of disease in infants is the mother's knowledge. Mother's knowledge about the signs and symptoms of ARI determines the mother's attitude in preventing and treating ARI in toddlers.Mother’s knowledge and good attitude can help in making efforts to prevent and treat ISPA in toddlers. Objective: To find out the description of the level of knowledge and attitudes of mothers towards ISPA in toddlers at the Tanah Jawa Health Center, Simalungun Regency. Methods: This research is a descriptive study with a cross-sectional study approach. Sampling in this study used a consecutive sampling technique with a total sample of 40 mothers who came to the Tanah Java Health Center. Data collection was carried out using a questionnaire. Results: From the research conducted, it was found that the level of knowledge of mothers about ARI in toddlers was in the category of less 27 people (67.5%), enough 9 people (22.5%), and good 4 people (10%). The attitude level of toddler mothers is in the less category 28 people (70%), enough 8 people (20%), good 4 people (10%). Conclusion: The majority of the level of knowledge and attitudes of mothers under five at the Tanah Java Health Center are in the less category.
