    Abstract
    Background Type 2 Diabetes Melitus is a chronic metabolic disorder that is associated with hyperglycemia due to interference of insulin function, that could cause multi-organ damage and dysfunction. Formation of Reactive Oxygen Species is an integral component of the pathophysiology of T2DM, due to its ability to cause oxidative stress, that plays a pivotal role in the progression and complications of T2DM, by damaging the protein component of cells. Bilirubin which is commonly acknowledged as a by-products of heme metabolism, exhibits an inverse relationship with T2DM complications, that is hypothesized due to its antioxidant capability. Therefore, the potential ability of bilirubin to suppress oxidative stress is intriguing to be investigated. Objectives to determine whether there is an association between the levels of total serum bilirubin with the degree of oxidative stress in T2DM patients at Prof. dr. H. Chairuddin Panusunan Lubis Hospital in Medan. Methods A cross-sectional study is conducted on 73 T2DM patients at Prof. dr. H. Chairuddin Panusunan Lubis Hospital in Medan with purposive sampling technique. Blood of the patients were then collected, and spectrophotometry analysis was then carried out to measure the level of total serum bilirubin and advanced oxidation protein products. Spearman’s rank correlation coefficient was performed to analyze the significance, power and direction of the correlation between the two variables. Results The spearman’s rank correlation coefficient resulted in ρ = -0,279 with p value of p<0,05. Conclusion. There was a significant negative monotonic relationship between the levels of serum total bilirubin with the degree of oxidative stress in T2DM patients at Prof. dr. H. Chairuddin Panusunan Lubis Hospital in Medan.
