Background. Based on the Statista Research Department, the number of smartphone users in Indonesia is estimated to reach over 190 million in 2023. Everyone now use smartphone, university students have been identified as one of the largest smartphone users. Several studies have shown the negative effects of excessive smartphone use on health, one of which is headache. Aim. This study aims to determine the relationship between the level of smartphone use and the incidence of primary headaches among medical students at Faculty of Medicine, University of North Sumatra Class of 2020. Methods. This research is an observational analytic study conducted with a cross sectional design. Researcher used total sampling method. Data were collected by distributing questionnaires directly to respondents. The questionnaires used were SAS-SV and HO K-H & Ong BK-C. The analysis used was univariate and bivariate analysis using chi-square. Results. This study involved 252 respondents, 77 (30.6%) of them experienced primary headaches. Among the 77 respondents, 26 (33.8%) had migraine and 51 (66.2%) had TTH. There were 159 (63.1%) respondents addicted to smartphone. The results of statistical analysis showed an association between the level of smartphone use and the incidence of primary headaches (p=0,001). Conclusion. There is a correlation between the level of smartphone use and the incidence of primary headaches among medical students at Faculty of Medicine, University of North Sumatra Class of 2020.
