Diabetes Mellitus type 2 (DMT2) is a chronic disease caused by insufficient insulin production or the body's inability to effectively absorb insulin. DMT2 has long-term impacts and can lead to organ damage. In Indonesia, DMT2 is the third leading cause of death after stroke and coronary heart disease. This disease is considered catastrophic as it requires high treatment costs and can result in serious complications. Patients with complications from DMT2 impose a significant social and financial burden on the country. Cost of Illness (COI) studies are used to measure the economic burden caused by DMT2, involving direct costs (medical treatment and medications) and indirect costs (loss of work productivity). The Internal Medicine Clinic at Malahayati Islamic Hospital in Medan is the focus of the study. The research sample consists of 92 patients, with 88 covered by BPJS and 4 being non-BPJS patients. The aim of this research is to provide relevant information to patients to assist them in making informed decisions regarding treatment, considering the economic factors involved. This study employs a quantitative descriptive research approach supported by qualitative data based on the patient's perspective. Data is obtained through medical records and interviews with respondents using questionnaires. The research results indicate that the majority of DMT2 patients in the Internal Medicine Clinic of Malahayati Islamic Hospital in Medan are female, elderly, and engaged in entrepreneurial occupations. Additionally, the research findings illustrate that the expenses incurred by general patients are higher than those covered by BPJS. This is evident in the total monthly cost for non-BPJS DMT2 patients amounting to Rp1.357.826, whereas the total monthly cost for BPJS-covered DMT2 patients is Rp284.080. Therefore, it is recommended that patients obtain health insurance (BPJS) to minimize the expenses incurred.en_US
