    Abstract
    Background. Maternal mortality rate is an assessment of the quality of obstetric services in a country or region. The maternal mortality rate is still relatively high in North Sumatra, reaching 195 deaths per 100,000 live births. According to WHO, bleeding is the most common cause of maternal death. One of the most common causes of postpartum hemorrhage is PAS. Hemorrhage occurs as a life-threatening complication. Objective. This study aims to determine the relationship between age, parity and history of cesarean section on the incidence of PAS at H. Adam Malik Hospital Medan in 2020-2022. Methods. This study was analytic-observational in nature using a cross-sectional research design. The research data is secondary data in the form of medical record data of H. Adam Malik Medan Hospital from 2020 to 2022 which contains the population of mothers who gave birth, maternal age at delivery, maternal parity and previous cesarean section history. The research subjects were taken with simple random sampling technique and totaled 84 people. Results. Statistical analysis was performed with univariate and bivariate chi-square tests. The results of univariate analysis showed that age > 32 years, a history of giving birth 3-4 times and a history of cesarean section ≥ 2 had the highest frequency. The results of bivariate analysis showed that there was a relationship between age and the incidence of PAS (p=0.016), a relationship between parity and the incidence of PAS (p<0.001) and a relationship between cesarean section history and the incidence of PAS (p=0.004). Conclusion. There is a relationship between age, parity, and history of cesarean section with the incidence of PAS at H. Adam Malik Hospital Medan in 2020-2022.
