Perbandingan Sensitivitas dan Spesifisitas Skor Intracerebral Hemorrhage dan Skor Intracerebral Hemorrhage Modifikasi dalam Memprediksi Mortalitas Pasien Stroke Perdarahan di Rumah Sakit Umum Pusat Haji Adam Malik Medan

Abstract

Background. Hemorrhagic stroke is a cerebrovascular disease that causes acute brain functional impairment as a result of obstruction of blood flow to the brain due to hemorrhage. Hemorrhagic stroke has a higher mortality rate than ischemic stroke. Mortality percentage can be calculated using ICH score and modified ICH score. Objectives. This study aimed to compare the sensitivity and specificity of the ICH score and modified ICH score in predicting mortality of bleeding stroke patients. Methods. This study was an analytic study with a retrospective approach and used the total sampling method. The subjects of this study were hemorrhagic stroke patients at Haji Adam Malik Hospital who were admitted to the hospital during the period January 2022 to April 2023. The data used were secondary data obtained from medical record data. The analysis used was univariate analysis and ROC curve. Results. In this study, there were 69 samples eligible, of which 44 patients (63.8%) died and 25 patients (36.2%) survived during 30 days of hospitalization. ICH score ≥ 2 and modified ICH score ≥ 3 predicted 30-day mortality with the sensitivity and specificity rate of 68% and 84%, and 81.81% and 80%, respectively. Conclusion. The modified ICH score has a higher sensitivity rate than the ICH score, but has a lower specificity rate than the ICH score.
