Background. The prevalence of physical and psychological stress among medical students is high due to academic stressors, which is considered one of the main risk factors for the occurrence of Irritable Bowel Syndrome (IBS). Objective. To determine the relationship between stress levels and the risk of IBS occurrence in final-year medical students at USU Faculty of Medicine. Methods. Analytical approach with a cross-sectional design and stratified random sampling with a 1:1 male-to-female ratio among final-year medical students at USU Faculty of Medicine. Data were collected through questionnaires distributed using Google Forms. The questionnaires included the Perceived Stress Scale (PSS), IBS occurrence based on Rome IV Criteria, and Bristol Stool Form Scale (BSFS). Results. Out of 118 samples, 6 (5.1%) had mild stress, 103 (87.3%) had moderate stress, and 9 (7.6%) had severe stress. The incidence of IBS was found in 9 individuals (7.6%), with IBS-D (55.5%) being the dominant type, followed by IBS-C (22.1%). Statistical analysis showed no significant relationship between stress levels and the occurrence of IBS (p = 0.201). Conclusion. There is no significant relationship between stress levels and the risk of IBS occurrence.en_US
