    Abstract
    Background : COPD is a major cause of morbidity and mortality worldwide. Changes in the V/Q ratio, excessive respiratory load, and nutritional deficiencies are some of the main mechanisms that occur during COPD exacerbations and cause type 2 respiratory failure. The purpose of this study was to determine the pattern of germs in COPD patients with severe exacerbations with type II respiratory failure in RSUP patients. H. Adam Malik Medan and antibiotics which are still sensitive according to the results of the sensitivity test. Method : The design of this study was a descriptive study based on secondary data on medical records of COPD exacerbations at RSUP H Adam Malik for a period of 3 years (1 January 2020 to 1 December 2022). Results : Out of a total of 60% COPD patients who experienced exacerbations during the study period, 15 types of bacteria were found consisting of 3 types of bacteria, the most common in patients with Severe COPD Exacerbations with Respiratory Failure 60% which were often found, including Klebsiella pneumoniae as much as 33% then Pseudomonas aeruginosa as much as 26.6% and Escherichia coli as much as 20%. Then the antibiotics that are still sensitive to the 3 most germs are meropenem and amikacin. Conclusion : The most common bacteria in COPD patients with severe exacerbations with respiratory failure were Klebsiella pneumoniae with 33%, then Pseudomonas aeruginosa with 26.6%. Then the antibiotics that were still sensitive to the most germs were meropenem and amikacin. While Empirical Antibiotics that are often used such as Ceftriaxone and Ciprofloxacine have a low level of sensitivity.
