Faktor - Faktor yang Mempengaruhi Mortalitas Pasien Penyakit Paru Obstruktif Kronis dengan Gagal Napas Tipe II di RSUP H Adam Malik

Abstract

Background : Chronic obstructive pulmonary disease (COPD) is a health problem with significant morbidity and mortality rates, but until now the risk factors that affect COPD, especially type II respiratory failure, have not been clearly defined.. Methods: Observational research with a kohort retrospektif design using medical records data of patients diagnosed with chronic obstructive pulmonary disease dan type II respiratory failure treated at H. Adam Malik General Hospital Medan for 3 months period from November 2021 to September 2022. Correlation between the independent and dependent variables was done by Chi Square test. To obtain the independent variable that has the most dominant effect on the dependent variable, multiple logistic regression tests was used. Results: No significant relationship was found between gender, age, number of comorbidities, smoking habits and history of exacerbations with mortality rate of COPD patients (p>0.05). Patients who do not routinely use inhalers tend to be more at risk with a mortality rate of 19 times greater than patients who use inhalation therapy regularly. In addition, the longer the duration of care for COPD patients with type 2 respiratory failure, the lower the chance of experiencing mortality. Conclusion : Factors that influence mortality are routine use of inhalers and length of stay.
