    Abstract
    Background : Heart failure (HF) is a complex clinal manifestation that cause abnormality of structural and/or functional of the heart. HF is due to diastolic dysfunction and systolic dysfunction of the left ventricle (LV). The aim of this study is to assess the relationship between grading of LV diastolic dysfunction and quality of life in heart failure with reduced ejection fraction (HFrEF) patients. Method: This is a cohort retrospective study in HFrEF patients which are treated at Pusat Jantung Terpadu RSUP H. Adam Malik Medan on January 2022 until December 2022. Echocardiography has been done to assess LV diastolic dysfunction and then we measured the quality of life using MLHFQ, KCCQ-12, dan EQ-5D-3L by phone at 6-12 months after the time of echocardiography was done. Results: From 96 patients, there are 56 patients (58.3%) with grade I LV diastolic dysfunction, 12 patients (12.5%) with grade II LV diastolic dysfunction and 28 patients (29.2%) with grade III LV diastolic dysfunction. From the MLHFQ and KCCQ-12, there are 77 patients (80.2%) with good quality of life and 19 patients (19.8%) with poor quality of life. From the EQ-5D-3L, there are 44 patients (45.8%) with good quality of life and 52 patients (54.2%) with poor quality of life. There is a significant relationship between grading of pre-discharge LV diastolic dysfunction and quality of life measured by MLHFQ (p value 0,04) but not statistically significant when using KCCQ-12 and EQ-5D-3L (p-value 0,873 dan 0,856). Conclusion : Grading of pre-discharge LV diastolic dysfunction on HFrEF patient is correlated with quality of life in HFrEF patients.
