Background: Seborrheic keratosis (SK) is a benign epidermal tumor with clinical manifestations in the form of macules to papules stuck-on in areas exposed to sunlight such as the face. Seborrheic keratosis can be a cosmetic problem that is quite disruptive for some people, especially women. Fibroblast growth factor receptor-3 mutations in the epidermis layer in SK can cause proliferation in keratinocytes and activate anti-apoptotic proteins. Soursop (Annona muricata L.) is widely cultivated in Indonesia. Soursop seeds contain Annonaceous acetogenins which have anti-cancer effects, namely cytotoxicity, induction of apoptosis, necrosis and inhibition of proliferation and contain various antioxidants which result in clinical improvement of SK. Objective: To determine the effect of 0,3% soursop seed extract cream (Annona muricata L.) on SK. Subjects and Methods: This quasi experimental clinical trial study was conducted with a pretest-posttest design on 30 subjects with the common seborrheic keratosis variant of SK. Diagnosis was made through history and clinical examination. Evaluation of clinical improvement using a digital caliper tool before and after administration of 0.3% soursop seed extract cream were carried out at weeks 0, 4, and 8. Side effects were observed during the study and satisfaction was assessed at the end of the study. The Friedman test followed by a post-hoc test using the Wilcoxon test was performed to assess the difference in lesion area before and after treatment with a p<0.05 value considered significant. Results: There was a significant decrease in the mean of the lesion size as well as the lesion’s area differences in subjects with SK (p<0.001) after 8 weeks of application of 0.3% soursop seed extract cream. No side effects were observed and all subjects were satisfied with the application of 0.3% soursop seed extract cream (Annona muricata L.). Conclusion: The use of 0.3% soursop seed extract (Annona muricata L.) cream can improve clinical appearance of SK, without side effects and good satisfaction.en_US
