Clean water and sanitation are one of the most basic human needs and also an important part of improving human health. The problem of clean water and sanitation is a problem that often occurs throughout the world, especially in developing countries. The sanitary conditions of residential areas and the quality of clean water used by the people of Aur Village are very worrying. This can cause health complaints in the community. This research aims to analyse the relationship of residential sanitation and clean water quality with community health complaints in Aur Village, Medan Maimun Sub-district. This type of research is quantitative research where assessments are carried out on sanitation requirements, water quality and health complaints using primary and secondary data sources. This research was conducted in the Deli River basin in Aur Village, Medan Maimun Sub-district, Medan City from February 2024 to May 2024. The population in this study was 1,598 heads of families. The research sample determined using the Slovin formula was 97 respondents using a purposive sampling technique. The research results showed that 87.6% of houses did not meet residential sanitation requirements. 86.6% of PDAM water does not meet physical quality requirements. The biological quality of PDAM water meets the requirements with a result of 0 CFU/100 ml, while the Deli River water does not meet the requirements with a result of 45 CFU/100 ml. As many as 82.5% of people experienced health complaints. There is a significant relationship between residential sanitation and community health complaints with a value of P=0.001. The government needs to relocate the people of Aur village to residential locations that are liveable, healthy and in accordance with regulations because Aur village is in the river protection boundary area which is an area that does not have permits for residential areas
