    Abstract
    The health problems that are still common in Indonesia in recent years are non- communicable diseases. Non-communicable diseases is a disease that often goes undetected because there are no complaints and no symptoms and is usually found at an advanced stage so it is difficult to cure and ends in disability or death. The prevalence of hypertension in the city of Medan in 2018 was 7,107 people. If this disease is not controlled properly, it can increase the prevalence rate and continue to increase. This research is a qualitative research with a case study research design, determining informants was carried out using a purposive sampling technique. The results of the research show that the input for implementing the hypertension control program at the Hamparan Perak Community Health Center includes inadequate human resources, facilities/infrastructure and funding, the program implementation process is guided by books published by the Ministry of Health, with the percentage of visits by hypertensive patients still not being achieved. . The availability of integrated health post non-communicable diseases facilities and infrastructure is still insufficient. There is a lack of other facilities and infrastructure at integrated health post, such as the absence of a participant monitoring book during the implementation of integrated health post non-communicable diseases so that monitoring of risk factors for non-communicable diseases in participants is not carried out, but another alternative is to record it in the integrated health post visit book. Apart from that, it is also hoped that the local sub-district can do this. budgeting for integrated health post non-communicable diseases in terms of procurement of infrastructure so that infrastructure can be sufficient during the implementation of integrated health post and activities for early detection of risk factors for non-communicable diseases can be carried out optimally
