    Abstract
    Basic sanitation is a basic effort that must be made to improve public health by providing a healthy environment that meets health requirements, including the provision of clean water, healthy latrines, waste management and sewerage. Based on the environmental health angle, unhealthy food has a huge influence on health because food or drink can act as a disease agent. According to the Indonesian Minister of Health Decree No. 1098/MENKES/SK/VII/2003 on Hygiene Sanitation Requirements for Eateries and Restaurants, flies are one of the vectors of disease transmission. The occurrence of disease transmission events caused by flies comes from food originating from food management sites (PTM), especially catering services, restaurants whose management does not meet health or environmental sanitation requirements. To get food and beverages that meet health requirements, it is necessary to supervise the basic sanitation of food and beverages that are prioritised in public businesses such as one of them, namely restaurants. The purpose of this study was to determine the basic sanitation and fly density in restaurants. This type of research is a descriptive survey. The sample in this study was the entire population in the study, namely four restaurants on Jalan Dokter Mansyur Medan City. The results showed that the condition of clean water supply and wastewater management had met the requirements (100%) but the condition of latrines and waste management did not meet the requirements. In addition, the level of fly density in restaurants was in the medium category (50%) and in the high category (50%). This study recommends that restaurant managers improve hygiene sanitation and control flies in an effective way according to the level
