dc.description.abstractBackgrounds: Diabetic retinopathy is a common microvascular complication of diabetes mellitus (DM). In Indonesia, diabetic retinopathy is the second most common complication after nephropathy. Retinal neurodegenesis may be an early indicator of the development of diabetic retinopathy. Tumor Necrosis Factor-Alpha (TNF-α) plays a role in the pathogenesis of inflammatory and neovascular disorders in the eye. TNF-α is associated with various intraocular inflammatory diseases, such as macular edema and proliferative diabetic retinopathy. Objective: To find the relationship between TNF-α levels and Retinal Nerve Fiber Layer (RNFL) thickness in diabetic retinopathy patients at Prof. CPL Universitas Sumatera Utara General Hospital and Affiliated Hospital. Method: This study used a cross sectional method which was carried out on 45 patients with type 2 DM and diabetic retinopathy at the eye clinic at Prof. CPL Universitas Sumatera Utara General Hospital and Affiliated Hospital from March 2024 to June 2024. The RNFL and posterior segment examinations were performed. TNF-α from blood samples. Blood sugar level data was taken from medical records. Results: A total of 45 people participated in this research. There were 24 men (53.3%) and 21 women (46.7%). 32 (71.1%) of the patients had DM for >5 years, 27 (60%) experienced PDR, the highest location for RNFL thickness examination was superior, 19 (42.2%). Average TNF-α with a thin RNFL thickness level was 65.67 ng/L, with a thick RNFL thickness level of 64.78 ng/L. The mean TNF-α with a thin superior RNFL thickness level was 65.74 ng/L, the thick one was 63.70 ng/L. The mean TNF-α with the thickness of the thin inferior RNFL was 65.73 ng/L, the thick one was 63.9 ng/L. The mean TNF-α with the thin temporal RNFL thickness was 68.73 ng/L, the thick one was 67.19 ng/L. The mean TNF-α with thick nasal RNFL thickness was 67.06 ng/L. Conclusion: There was no statistically significant relationship between TNF-α and RNFL thickness on the temporal, superior or inferior side in diabetic retinopathy patients.en_US
