Scabies is skin disease caused by Sarcoptes scabiei variant hominis mites and transmitted through skin contact and personal items. This study aims to determine the relationship of dormitory physical environment and scabies prevention behavior with scabies incidence at Mardhatillah Boarding School in 2024. This study was quantitative analytic with cross sectional design with a population of 636 people and sample of 84 selected using simple random sampling. The results of respondents characteristic showed that most respondent were male (57.1%), age 14 years (25.0%) and had MTs education (70.2%). Measurements of physical environment in 15 dormitory rooms showed all rooms had unqualified occupancy density, 13 rooms had unqualified humidity and lighting, 10 rooms had unqualified ventilations and 3 rooms had unqualified room temperature. Measurement of scabies prevention behavior in 84 respondents showed 64.3% had poor scabies prevention knowledge, 86.9% had good scabies prevention attitudes and 50% had good scabies prevention actions and 51.2% experienced scabies incidence. Bivariate analysis using Chi square test showed that variables associated with scabies incidence were humidity (p=0.030), lighting (p=0.030), knowledge (p=0.001) and action (0.000), while variables that were not associated with scabies incidence were ventilations (p=0.890), room temperature (p=0.118) and attitude (p=0.089). The occupancy density was not found to be related because the measurement results were homogeneous. Islamic boarding schools are advised to pay attention to the dormitory environment by limiting numbers of students, adding fans and maximizing lighting, for students to improve scabies prevention efforts, and health office to educate students about scabies prevention
