    Abstract
    The Universal Health Coverage (UHC) concept was formed with the aim of providing affordable and good quality health services for everyone without causing financial difficulties for individuals. Regional governments have a very significant role in optimizing the implementation of UHC in their regions. This research aims to analyze the level of success in implementing Universal Health Coverage (UHC) in Medan City to improve health services seen from the perspective of regional autonomy. This research uses descriptive research methods with a qualitative approach, data collection techniques namely interviews, observation and documentation. This research was located at the Medan City Health Service with research informants namely: Head of Health Services, PIC of the UHC JKMB program, DPRD, PIC BPJS Hospital Prof. Dr. Boloni Medan, PIC Health Promkes Medan Area South Health Center, and the community. The data and information collected were then analyzed based on four indicators used to measure the success of regional autonomy, namely Yosef Riwu Kaho's regional autonomy indicators in Martin (2007) which were put forward through human implementers, financial capabilities, equipment and supplies, and organizational management. The research results confirm that through implementing human indicators, the Medan City Government has a strong commitment to the health of its people by realizing a health service program that provides access to services using E-KTP. Through equipment and supplies indicators, the Medan City Government also facilitates access to UHC JKMB program services with several hospitals outside Medan City to provide wider service coverage and maximize health workers to provide optimal health services. Furthermore, through financial capacity indicators, the budget allocation for the health sector is 18.71% of the total 2023 APBD, which exceeds the minimum stipulated limit of 10%, reflecting significant dedication in strengthening the health sector. Then, through organizational and management indicators, collaborative efforts between OPDs have also shortened the hierarchical level in decision making, increased bureaucratic efficiency, and accelerated responses to community needs. Thus, the implementation of regional autonomy in Medan City shows a real improvement in achieving UHC goals. However, it is necessary to develop online-based information system services that can be accessed in real-time as well as increase outreach to the public to ensure wider understanding and participation in this program.
