Background: Total abdominal hysterectomy is the most common performed technique; however, vaginal hysterectomy is considered less invasive and associated with lower complication rates. This study aimed to compare the quality of life of women following abdominal versus vaginal hysterectomy. Methods : This study was an analytical observational study with a cross-sectional design, conducted in Haji Adam Malik Central General Hospital and satelite hospitals in Medan. This study included women aged 30 to 60 years who had undergone total abdominal hysterectomy (TAH) or total vaginal hysterectomy (TVH) within the last 3 to 6 months. Women who had undergone cesarean hysterectomy, hysterectomy for malignancy, or had metabolic disorders such as hypertension and diabetes were excluded. Quality of life was assessed using SF-36 questionnaire. Results: We conducted a total of 60 subjects met the inclusion criteria, with 30 women in the TAH group and 30 women in the TVH group. The TVH group experienced significantly less bleeding (p < 0.001) and shorter operative times (p < 0.001), but there was no significant difference in the incidence of complications (p = 0.112). The TVH group scored higher in domains such as physical functioning, physical limitations, bodily pain, general health, vitality, social functioning, emotional limitations, and mental health compared to the TAH group. However, there was no significant difference in the overall quality of life score between the two groups. Conclusion: This study suggests that there is no significant difference in the overall quality of life between women undergone TAH or TVH procedures.en_US
