Objectice:
To determine the value of troponin T and troponin I diagnostic tests on systolic and diastolic function in children with sepsis.
Method:
This research is a retrospective cohort study at Haji Adam Malik General Hospital Medan from March 2021 to September 2021. Subjects were selected by consecutive sampling. Blood samples were taken to check the initial data from the PELOD-2 Score. The patient also underwent an ECG and chest X-ray examination and an echocardiographic examination was carried out in each child with sepsis on the first and third days. Data were analyzed using diagnostic test analysis statistics. From the data we will look for cut off troponin T and troponin I, receiver operator curve (ROC) and areas under the curve (AUC), and calculate sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV).
Result:
The study was followed by 30 children with sepsis in the pediatric intensive care unit at Haji Adam General Hospital who met the inclusion criteria. Most of the subjects were male with 21 people (70%). The mean age of children is 7.9 years. In this study, it was found that there were 8 children (26.7%) with impaired diastolic function on the first day and 11 children (36.7%) on the first day. Troponin I had a sensitivity of 75%, a specificity of 81.8% in assessing impaired diastolic function. On the third day of treatment, a sensitivity value of 72.7% was obtained, a specificity of 78.9%. Troponin T on the first day, obtained a sensitivity value of 87.5%, a specificity of 77.3%. on the third day of treatment, a sensitivity value of 81.8% was obtained, a specificity of 73.7%.
Conclusion:
Troponin I and Troponin T can be used as predictors of impaired diastolic function in pediatric patients with sepsis.
