Abstract

Hemodialysis is a therapy for patients with end-stage renal failure. There are several changes in elderly patients who carry out hemodialysis therapy, one of which is social frailty. Social frailty in the elderly has many factors, namely chronic diseases experienced, lack of ability to carry out daily activities, and the emergence of loneliness and loss of loved ones. This study aims to determine the social frailty of Mr. T with a case study method. T with the case study method. Nursing care services and implementation in elderly patients with CKD on Regular HD are carried out with NANDA, NIC, NOC, ISDA, and ICRM can maximize the performance of nurses in providing nursing care. The diadnose that is established by NOC from the risk of weak elderly syndrome is self-management: chronic disease, insomnia, namely sleep and the risk of loneliness, namely the severity of loneliness. With NIC the risk of weak elderly syndrome; Self-care assistance, Insomnia; Improved sleep, and Risk of loneliness; Improved family integrity.
