    Abstract
    Indonesia has experienced an increase in the prevalence of chronic kidney failure patients undergoing hemodialysis therapy. There were at least 77,892 active patients undergoing hemodialysis in 2017, then there was an increase of up to 41% to 132,142 patients in 2018, the number of new patients doubled compared to 2017. In 2018, the North Sumatra region had 4,076 new patients, of which in in 2017 the number of new patients was 2,690. The 2018 IRR results stated that the largest proportion of patients were in the 45-64 year category. The purpose of this writing is to describe nursing care for patients with CKD on HD Regular with hypertension in hospital. Prof. Chairuddin P. Lubis, University of North Sumatra. In the study using the ISDA flow (Intan’s Screening Diagnoses Assesment) to help assess clients and establish a diagnosis with NANDA (North American Nursing Diagnosis Association). Based on the identification of nursing diagnoses, ICRM (Intan’s Clinical Reasoning Model) used for the convenience of NOC determination (Nursing Outcome Classification) and NIC ( Nursing Intervention Classification) without knowing the exact etiology based on the case. The results of the assessment and data analysis carried out revealed problems, the risk of ineffective renal perfusion and excess fluid volume, with the NIC intervention given, namely, hemodynamic regulation and hypervolemia management. It is hoped that the results of this paper can become a new reference in the realm of knowledge related to the problem CKD on HD Regular with hypertension.
