Abstract

Frailty syndrome in the elderly is a clinical condition characterized by increased vulnerability to various stressors, resulting from a decline in multi-organ physiological function due to aging. Elderly individuals in Indonesia, aged >=75 years, occupy the top ranking for the group of chronic kidney disease (CKD) patients. Chronic kidney disease (CKD) is kidney damage characterized by a decrease in the glomerular filtration rate (GFR) of less than 60 mL/minute/1.73 m² for a minimum of 3 months. Risk factors include age, diabetes mellitus, family history of chronic kidney disease, history of hypertension, history of smoking, and alcohol consumption habits. This study aims to apply the concept of nursing care to elderly chronic kidney disease patients. The research was conducted using a case study method with a nursing process approach, including assessment, problem analysis, nursing intervention, nursing implementation, and nursing evaluation. Nursing diagnoses identified in Mrs. R based on the assessment using ISDA were: sleep pattern disturbance and body image disturbance. ICRM, NOC, and NIC were used as screening tools for interventions and outcomes based on established nursing diagnoses.
