Knee osteoarthritis (OA) is a degenerative joint disease characterized by cartilage damage, leading to chronic pain, stiffness, and functional limitations. This study aimed to analyze the effects of manual traction physiotherapy on serum levels of C-terminal Telopeptide of Type II Collagen (CTX-II), a biomarker of cartilage degradation, and the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) scores. This experimental one-group pretest-posttest study included 36 knee OA patients. CTX-II levels were measured using the ELISA method, while WOMAC assessed pain, stiffness, and physical function. The results revealed a significant reduction in total WOMAC scores post-intervention (p < 0.001), including improvements in pain, stiffness, and physical function. However, CTX-II levels showed no significant change following the intervention (p = 0.504). Correlation analysis between changes in CTX-II levels and WOMAC scores indicated no significant relationship (p = 0.565). These findings suggest that the observed clinical symptom improvements may be attributed more to neurophysiological mechanisms than to structural changes in cartilage. In conclusion, manual traction effectively alleviates clinical symptoms of knee OA, although its impact on cartilage degradation biomarkers requires further evaluation over a longer intervention period.en_US
